Please Include the following information:
1) Your pets name.
2) Your first and last name (Your last name
will not be mentioned).
3) Your email address. (For Pampered Pets Only - Your email address will not be displyed)
4)
Your state.
A voting ballet link will be available starting on November 1st on our website as well as on our
facebook page so you can cast your vote for your favorite pet photo.
The winner will recieve a $25.00 gift
certificate from Pampered Pets Catalog and will be notified via email. The winners name and pet photo will be posted on our
website and on our facebook page....It could be you!
